You can download our Content licence form (PDF) (59 KB; 6 pages) and a simple guide to filling in the form (PDF) (27 KB; 3 pages).
You can fill the form in by hand or electronically. Return your form to email@example.com (or by post) and we will consider your request and complete our areas of the form. Alternatively, if the licence relates to a map, please send the form to firstname.lastname@example.org.
You may not need to complete a Content licence form to re-use material in some cases. Many digital images we publish online may be re-used for commercial and non-commercial purposes without a Content licence, as indicated by any rights statement or licence we have applied. See our Permission to use material page and PSI information page for details.

- How many products you are going to make (if applicable)
- What sort of products or services you are going to use the content for

What happens next
When you return the licence we will check the details and whether the material you have requested is reusable. We will enter further details on the form if needed, such as the permissions fees that may be payable.
